Reach Out Mentoring Program

Mission of Mentoring Programs

The program's mission is to provide the elderly, disabled, terminally ill, chronically ill, homebound, and their families programs to assist in promoting a state of well-being.  The program aims to link individuals with resources in the community, to rekindle their creativity giving them a sense of self-worth, and to help build sufficiency, and in some cases, the sheer satisfaction of achievement.

Mentoring Programs:

  • Mentoring (One on One) of the elderly, the homebound, terminally ill and the disabled.  Mentors are trained by LINK's collaborative partners with, and are matched, with clients to provide home visits, reassuring phone-calls, to help fill out forms, to read mail, to run errands such as picking up groceries or medicines, to support them, and to help the client with access to other services in the community.

What is the LINK’s ReachOut Mentoring Program?

LINK’S Mentoring Program was founded in 1997 through a generous grant from the Robert Wood Johnson Foundation/Faith in Action grant, The Bernardine Franciscan Sisters Foundation, Inc. and the Unitarian Universalist Funding program. As a network of community agencies, civic organizations, homemakers, business and interfaith congregations the program works in partnership training volunteers to become mentors who provide one-on-one, guidance, support, assistance, education and referral services to people whose circumstances leave these needs unmet.
